Our 6-step process to comprehensive domiciliary and supported living care

If you choose Peace Nest Care Homes for your domiciliary or supported living home care in Staffordshire and Derbyshire, we’ll follow a clear and simple 6-step process to ensure we deliver the very best care to meet your particular needs:

The Care Assessment 01

The Care Assessment

We’ll carry out a comprehensive care and support assessment, assessing mobility, medication, allergies, nutrition, bathing needs and more. This gives us a complete picture of a resident's needs and requirements.
The Property Risk Assessment 02

The Property Risk Assessment

The safety of our service users and our carers is paramount, which is why we assess every resident's property for potential hazard in domiciliary care.
The Personal Profile 03

The Personal Profile

A big part of delivering truly effective care is getting to know our residents interests, hobbies and involvement with the community. We, therefore, have a good chat with residents and/or their family members so we can learn what is important to them on a more personal level.
The Support Network 04

The Support Network

Our goal is to deliver effective care that complements and supports the care already being received by our residents. That’s why we’ll identify the key individuals who provide support and care to our residents, both professionals and relatives, and determine how often this support is provided. This ensures we can all work together to provide joined-up care.
The Care Plan 05

The Care Plan

Based on all of the information we have gathered, we’ll put together a tailored care and support plan for the individual resident. This plan is not set in stone and will be updated and adapted to meet the changing needs of the resident.
Carer Assignment 06

Carer Assignment

We value the importance of developing trusting relationships between carers and their resident. To facilitate this, we assign a consistent team of carers to deliver personalised care to each of our resident, giving everyone involved the chance to build lasting relationships.

The carers at Peace Nest Care Homes are brilliant! They give 100% and go the extra mile for my dad. Everything he needs done, is done to the highest standard.”

Mr C Hayley and daughter Emily.

How to Apply

Some of our services accept direct applications, also known as self-referrals, allowing you to contact the local team at your chosen service and apply directly. However, most of our supported living services operate under agreements with the local authority, meaning a referral or application must be made on your behalf by their team.

Once we receive your application, a member of our friendly team will get in touch to discuss the next steps and arrange an assessment of your needs.

Use our property search to explore available services near you, check eligibility criteria, and learn how to apply. You must meet all eligibility requirements for your application to be considered. Your local authority can also provide guidance on housing and support options available in your area.
